Home Health Physical Therapist

About the role

At Ascension at Home, together with Compassus, we are dedicated to caring for our teammates as the first step in caring for our patients. As a Home Health Physical Therapist you will work one‑on‑one with patients in their homes, helping them gain the strength and independence needed for daily activities such as attending church or gardening.

Responsibilities

• Provide physical therapy services directly in patients’ homes.
• Assess, plan, and implement individualized treatment plans to improve mobility, strength, and functional independence.
• Educate patients and caregivers on exercises, use of durable medical equipment, and safety strategies.
• Document all clinical encounters accurately and timely.
• Collaborate with interdisciplinary team members to coordinate comprehensive care.

Qualifications

• Bachelor of Science in Physical Therapy.
• Current state license as a Physical Therapist.
• Valid driver’s license.
• Current CPR certification.
• Negative TB screen and Hepatitis consent/declination.
• Minimum two years of experience as a Physical Therapist in an acute care or rehabilitation setting.

Skills
