Through personalized, goal-oriented care, you'll empower patients to overcome limitations, improve function, and enhance their quality of life. Responsibilities Plan and administer prescribed physical therapy treatments in patients' homes to help them achieve their highest level of physical functionAssess and document patients' physical strengths, limitations, and progress to support physician evaluation and treatment planningDevelop and implement individualized rehabilitation programs consistent with the physician's plan of care and patient goalsProvide clear care instructions to Physical Therapist Assistants and Home Health Aides and oversee their performance and documentationAccurately document patient assessments, interventions, and outcomesCommunicate patient progress to physicians, the Clinical Manager, interdisciplinary care team, patients, and familiesComplete and submit all required evaluations, treatment plans, progress notes, and discharge summaries in a timely manner Required Qualifications Current, unrestricted Physical Therapy licenseCurrent CPR certificationExperience collaborating with an interdisciplinary healthcare team Preferred Qualifications One year of experience as a Physical TherapistPrevious home health experience preferred Additional Information TB Screening Requirement This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. Candidates selected for this position will be required to complete TB screening. Driving Requirement This role is part of Humana's driver safety program and requires a valid state driver's license and personal vehicle liability insurance. Associates must maintain insurance coverage that meets state minimum requirements or Humana's minimum requirements, whichever is greater.
