About the role

We are seeking a Home Care Physical Therapist to deliver person-centered therapy services to adults in their homes.

Responsibilities

• Evaluating functional mobility and developing evidence-based treatment plans
• Delivering interventions aimed at promoting safety, independence, and quality of life
• Collaborating with an interdisciplinary team to ensure comprehensive care
• Documenting patient outcomes and progress
• Educating patients and caregivers on therapy strategies and safety measures

Qualifications

• Licensure as a Physical Therapist
• Strong assessment and treatment planning abilities
