Home Health Physical Therapist Assistant PTA

About the role

All About Home Care is a Houston-based home health care agency serving Houston and surrounding communities since 2004. We are currently looking for a Physical Therapist Assistant (PTA) to care for patients in the Southeast part of Greater Houston including Baytown, Pasadena, La Porte, Pearland, Friendswood, League City, and Texas City. We are also seeking PRN PTAs for The Woodlands and Conroe areas. Documentation is managed using Kinnser software.

Responsibilities

• Understands and adheres to established policies and procedures
• Provides physician-prescribed physical therapy under a plan of care established by a PT
• Improves or minimizes residual physical disabilities of patients
• Returns the individual to optimum and productive level within patient's capabilities
• Performs all skilled procedures as ordered by physician and according to the plan of care established by a PT
• Participates with all other health care personnel in patient care planning
• Takes initial history and performs initial evaluation
• Consults with PT regarding change in treatment
• Instructs patients and family/significant others in home programs and activities of daily living
• Participates in mandatory in-service programs and presents in-service programs as assigned
• Participates in performance improvement activities as assigned
• Attends all patient care conferences as scheduled
• Prepares medical records, progress notes, and updates care plans for each patient visit in a timely manner as per agency requirements

Qualifications

• Graduate from a two-year college level program approved by the American Physical Therapy Association
• Currently licensed in the state of Texas
• Two years of experience in Home Health preferred
