Job Description

All About Home Care is a Houston‑based home health care agency. We’ve been serving Houston and the surrounding communities since 2004, providing quality home health services. We are currently looking for Physical Therapist Assistant (PTA) to help care for patients in the Southeast part of the Greater Houston including areas such as Baytown, Pasadena, La Porte, Pearland, Friendswood, League City, and Texas City. We are also seeking PRN PTAs for The Woodlands and Conroe areas. We use Kinnser software to document patient care.

Qualifications

• Graduate from a two‑year college level program approved by the American Physical Therapy Association
• Currently licensed in the state of Texas
• Two years of experience in Home Health preferred

Responsibilities

• Understand and adhere to established policies and procedures
• Provide physician‑prescribed physical therapy under a plan of care established by a PT
• Improve or minimize residual physical disabilities of patients
• Return the individual to optimum and productive level within patient’s capabilities
• Perform all skilled procedures as ordered by physician and according to the plan of care established by a PT
• Participate with all other health care personnel in patient care planning
• Take initial history and perform initial evaluation
• Consult with PT regarding change in treatment
• Instruct patients and family/significant others in home programs and activities of daily living
• Participate in mandatory in‑service programs and present in‑service programs as assigned
• Participate in performance improvement activities as assigned
• Attend all patient care conferences as scheduled
• Prepare medical records, progress notes, and update care plans for each patient visit in a timely manner as per agency policy
