Key Responsibilities
- Evaluate patients and develop individualized treatment plans
- Provide occupational therapy services according to the established Plan of Care
- Assess patient progress and modify treatment plans as needed
- Document evaluations, treatments, and patient outcomes accurately
- Educate patients and caregivers on adaptive equipment, home safety, and therapeutic exercises
- Collaborate with physicians and the interdisciplinary care team
- Maintain compliance with all state, federal, Medicare, and agency regulations
Qualifications
- Active Florida license as an Occupational Therapist (OT)
- Minimum of 1 year of home health experience preferred
- Strong clinical assessment, documentation, and communication skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation and a valid driver's license
Preferred Skills
- Experience with electronic medical records (EMR)
- Knowledge of Medicare guidelines and home health documentation standards
- Bilingual (English/Spanish) is a plus
