Description
The home health Occupational Therapist at Assisteo Home Health plans, organizes, and directs home care services in the patient's home environment. Services can be provided in an independent home, assisted living facility, or group home. The therapist must be experienced in occupational therapy, with an emphasis on community health education/experience. They build from community resources to meet individual and family needs through planned visits.
Essential Job Functions/Responsibilities
- Provides direct patient care.
- Completes an initial assessment of the patient and performs a medication reconciliation.
- Assumes responsibility to coordinate patient care.
- Develops a care plan, establishing goals based on clinical diagnosis and incorporating therapeutic, preventive, and rehabilitative care.
- Communicates with physicians/nurse practitioners regarding the patient's needs and reports any changes in the patient's condition.
- Obtains/receives orders as required to coordinate the plan of care.
- Understands and practices all State and Federal Laws and their scope of practice.
Position Qualifications
- Occupational Therapy license to practice professional therapy in Arizona or a qualifying compact state.
- Home health experience is preferred.
- One year experience in nursing is mandatory.
- Must have a valid driver’s license and be able to work in the United States.
- Must have computer skills and be able to learn to use Electronic Medical Records Systems.
- Demonstrates excellent observation, problem-solving, organizational, verbal, and written communication skills.
- Must be self-motivated and able to work independently.
- Current CPR certification.
- Current fingerprint clearance card.
- Reliable vehicle with current auto insurance.
- Good standing with the Department of Motor Vehicles.
- Must be able to pass a background check for pre-employment.
Working Environment/Physical Requirements
- Prolonged or considerable walking, standing, and driving.
- Must be able to lift, position, or transfer patients and lift supplies/equipment.
- Considerable reaching, stooping, bending, and kneeling.
- Needs visual acuity, hearing, dexterity, and attention to detail while performing required nursing skills.
- Must have ability to perform CPR.
