Home Health Occupational Therapist- Per Diem

Boost Home Healthcare is a Medicare-certified, ACHC-accredited agency in Brookline seeking an exceptional OT to join our ground-floor team. This role offers supplemental income without the red tape of a large corporation. Work locations include Boston, Cambridge, Brookline, Newton, Needham, Wellesley, Quincy, and Milton.

True Flexibility: No quotas. Tell us your availability; we match the visits. Direct Access: Work directly with our Director of Nursing, not layers of management. Transparent Pay: Published per-visit rates ($70–$110) plus travel reimbursement. Clinical Integrity: Solid infrastructure with a small-team, "ground floor" feel.

Responsibilities

• Conduct initial OT evaluations and develop individualized plans of care under physician orders
• Provide skilled occupational therapy focused on ADL retraining, adaptive equipment, home safety, and functional independence
• Reassess patient progress and modify plans of care every 30 days
• Complete OASIS documentation accurately and on time in compliance with Medicare and ACHC standards
• Coordinate with the clinical team and patient's physician on care planning; supervise COTAs as needed per MA practice standards

Requirements

• Active Massachusetts OT license in good standing
• Graduate of an ACOTE-accredited occupational therapy program
• 1+ year of home health or acute care experience preferred; motivated candidates with strong clinical backgrounds considered
• Familiarity with Medicare OASIS documentation (training available)
• Reliable transportation, valid MA driver's license, and current auto insurance
• Available for at least 2–4 visits per week (flexible on timing)

$70.00 – $110.00 per visit
