Key Deliverables

  • Assists the physician in evaluating the patient's level of function.
  • Affixes patient's home environment for safety hazards and architectural barriers, identifies equipment needs.
  • Identifies functional problems and establishes goals and a plan of care designed to improve the patient's functional status (revising as necessary).
  • Performs patient tests, measurements, and evaluations, such as range of motion and manual muscle tests, gait and functional analysis, and body parts measurement, and records and evaluates findings to aid in establishing or revising specifics of treatment programs.
  • Performs and completes start-of-care, and ongoing assessments and associated paperwork in accordance with agency policy.
  • Periodically re-evaluates and upgrades the patient's program to maximize functional improvement.
  • Administers the exercise program utilizing specialized therapeutic exercise techniques and equipment.
  • Plans and administers prescribed physical therapy treatment programs for patients to restore function, relieve pain and prevent disability following disease, injury, or loss of body parts.
  • Recommends prosthetic/orthotic devices for patients or evaluates their devices and makes minor adjustments or recommends modifications by the prosthetist/orthotist.
  • Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care.
  • Performs and submits required documentation of patient's evaluations, plan of care, clinical notes, and progress reports, summaries along with changes in orders.

Tactical Deliverables

  • Notifies patient/clients prior to the visit and communicates further visits scheduled.
  • Drives responsibly and safely to and from the client's home.
  • Schedules own time based on priorities and maintains productivity utilizing time management.
  • Provides appropriate and thorough service to patients/clients, ensuring proper completion of services in an accurate and timely manner and in accordance with agency policy.
  • Makes the initial patient/client Start of Care (SOC) assessment within appropriate time as scheduled by the agency.
  • Always demonstrates respectable customer service and professionalism.
  • Establishes Home Bound Status and documents.
  • Provides skilled services and/or supervises, requiring substantial knowledge and skills in accordance with the Plan of Care (POC) and accepted standards of professional practice.
  • Determines discipline frequencies according to physician order and/or assessed needs.
  • Implements and adheres to physician's orders.
  • Obtains physician order or verbal orders for Start of Care (SOC) or Resumption of Care (ROC) or Recertification (RECERT) within twenty-four (24) to forty-eight (48) hours from the time of service.
  • Notifies physician with any changes with the initial plan of care (POC) and obtains any appropriate orders.
  • Provides teaching to patient/client and answers health-related questions and documents patient/client/caregiver understanding of education.
  • Completes Home Health Nursing Aide care plan and makes supervisory visits on Home Health Aides and PTAs at least every fourteen days (14).
  • Documents patient's/client's care using appropriate agency forms and submits to agency within appropriate timeline.
  • Absorbs any medication changes with each Re-visit and updates the Medication Profile Form in the home and in the agency's medical record.
  • Notifies physician with any change in client condition, abnormal findings and new medications.
  • Obtains orders as indicated with the appropriate form and submits them in a timely manner.
  • Completes Discharge Oasis and Discharge Summary when patient/client is discharged.
  • Completes Recertification Oasis every sixty (60) days while on service and obtains new orders for a new plan of care.
  • Communicates with the staffing coordinator with any scheduling concerns and/or changes in patient/client assignment.
  • Communicates all Start of Cares (SOC), Resumption of Care (ROC) or Recertification (RECERT) to the clinical support division within twenty-four (24) hours of completion.
  • Maintains continuity of care and documents communications with others within the health care team.
  • Demonstrates work that meets quality standards.
  • Aids in periodic auditing of paperwork of self/peers.
  • Aids in training of new personnel for field duty.
  • Communicates regularly with the Director of Nursing and Clinical Supervisor with all patient/client concerns or ideas for better patient/client outcomes.
  • Adheres to Agency Code of Conduct, Standards of Practice, and Patients/Clients Rights/Responsibilities.
  • Demonstrates professionalism in appearance, adhering to Agency dress code.
  • Accessible transportation to and from patients/clients visits.
  • Ability to lift 30 pounds, move intermittently throughout the workday, inclusive of sitting, standing and bending, pushing, pulling or moving objects and patients/clients.
