About the role
Provide personalized rehabilitation services in patients' homes as a Home Health Occupational Therapist.
Responsibilities
- Collaborate with physicians and care teams to develop and adjust therapy plans
- Participate in care coordination and discharge planning
- Train staff and family members in daily living skills
- Submit comprehensive reports
- Supervise therapy assistants and aides
Requirements
- Current unrestricted Occupational Therapy license in North Carolina
- Valid driver’s license with reliable transportation
- CPR certification
- Adherence to safety and TB screening protocols
Home health experience is preferred.
