You will work closely with patients, families, nurses, physical therapists, physicians, and other members of the interdisciplinary team to address functional limitations following hospitalization, illness, injury, or a decline in condition. This position is ideal for an OT who enjoys developing meaningful relationships with patients and providing individualized care in a one-on-one environment. Responsibilities Complete comprehensive occupational therapy evaluations and reassessmentsDevelop individualized plans of care based on each patient’s functional needs and personal goalsProvide skilled treatment focused on activities of daily living, upper-extremity function, strength, coordination, cognition, and safetyAssess home environments and recommend modifications, adaptive equipment, and assistive devicesProvide training related to bathing, dressing, toileting, meal preparation, and other daily activitiesEducate patients, families, and caregivers on safety strategies and therapeutic exercisesCoordinate care with nursing, physical therapy, physicians, and other providersParticipate in interdisciplinary communication and care planningComplete accurate and timely clinical documentationCommunicate changes in condition and safety concerns promptly Qualifications Current Connecticut Occupational Therapist licenseGraduate of an accredited occupational therapy programValid driver’s license and reliable transportationStrong communication, organization, and documentation skillsAbility to work independently while collaborating with the clinical teamA positive attitude and commitment to patient-centered care Home health experience is preferred, but we welcome occupational therapists with strong clinical skills who are motivated to learn and grow in the home health setting.
