Home Health Occupational Therapist

About the role

We are seeking an Occupational Therapist to deliver high-quality home health care services. The role involves collaborating with an interdisciplinary team to develop personalized care plans, evaluating and treating clients with physical and cognitive limitations, and recommending adaptive equipment and environmental modifications to enhance independence and safety.

The therapist will educate clients and caregivers on condition management and injury prevention, maintain accurate documentation, and stay updated on industry research and standards.

Responsibilities

• Collaborate with an interdisciplinary team to develop personalized care plans
• Evaluate and treat clients with physical and cognitive limitations

Requirements

• Active Occupational Therapy license in the state of Florida
• At least 1 year of relevant experience
• Strong communication, problem-solving, and critical thinking skills
• Ability to work independently and within a team
• Passion for patient-centered care
