Home Health Nurse- LPN/ RN
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
About the Client
Our client is a 63-year-old man who was in a motor vehicle accident in the 1990s. Currently in hospice and bed-bound, he lives in a clean, smoke-free home with his wife and three cats in Mt. Pleasant. He enjoys sports, watching TV, and playing games like solitaire, tic-tac-toe, and word searches.
- Care Requirements: Tube feeding, history of hypopituitarism, and multiple complex comorbidities.
- Safety Precautions: High risk for adrenal crisis, particularly from stressors like infection.
- Home Environment: Supportive family (wife is exceptionally kind and easy to work with), neat and clean home, three cats.
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care).
- Monitor and document patient condition and vital signs.
- Communicate with families and care teams.
- Maintain safe, high-quality, patient-centered care.
Schedule & Availability
We are looking to fill the following shifts:
- 9:00 AM – 5:00 PM on Saturday and Sunday.
Pay
- Weekly pay + optional daily pay.

Requirements
- Active RN or LPN license in the state of Michigan.
- Home care or private duty experience preferred (not required).
- Trach/vent experience a plus (training provided).
- Strong communication and critical thinking skills.
