Home Health Nurse- (LPN/ RN)

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care).
• Maintain a safe, high-quality, patient-centered environment.
• Coordinate with the Home Health Aide and family to assist with Activities of Daily Living (ADLs), including dressing and meal prep.
• Monitor and document vital signs and patient condition.

Requirements

• Active RN or LPN license in Michigan.
• Valid driver’s license with a clean driving record.
• Strong communication skills and a calm, patient temperament.
• Home care experience is a plus, but not required.
