Home Health Nurse- LPN/ RN

About the role

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting. Centria Healthcare is a Michigan-based home healthcare provider specializing in private duty nursing, complex pediatric care, and catastrophic injury services.

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care)
• Monitor and document patient condition and vital signs
• Assist with daily living activities (ADLs)
• Communicate with families and care teams
• Maintain safe, high-quality, patient-centered care

Qualifications

• Active RN or LPN license in the state of Michigan
• Home care or private duty experience preferred (not required)
• Trach/vent experience a plus (training provided)
• Strong communication and critical thinking skills

Pay

Weekly pay + optional daily pay; competitive pay based on experience.

Schedule

Full-time and part-time shifts available; day, night, and weekend shifts.
