Home Health Nurse- LPN/ RN
About the role
Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.
Client details: 25‑year‑old male, non‑mobile, resides in a hospital bed in the living room, requires ongoing tracheostomy, ventilator, and gastrostomy tube management. Diagnosed with Lennox‑Gastaut Syndrome, a severe form of epilepsy characterized by multiple seizure types that typically begin in early childhood.
Responsibilities
- Provide skilled nursing care (trach/vent care, medication administration, catheter care)
- Monitor and document patient condition and vital signs
- Communicate with families and care teams
- Maintain safe, high‑quality, patient‑centered care
- Manage tracheostomy, ventilator, and gastrostomy tube
- Support a clean, pet‑free home environment with a supportive family
Schedule
Full‑time and part‑time shifts available. Shifts are 6:00 AM – 6:00 PM and 6:00 PM – 6:00 AM, every day. Day, night, and weekend shifts are offered.
Pay
Weekly pay with optional daily pay available. Compensation is competitive and based on experience.

Qualifications
- Active RN or LPN license in the state of Michigan
- Home care or private duty experience preferred (not required)
- Tracheostomy/ventilator experience a plus (training provided)
- Strong communication and critical thinking skills
Patient Population
Pediatrics (birth–21 years) and adults with complex medical needs.
