Home Health Nurse I (RN) : HHH Home Health

Responsibilities

• Provides direct patient care in accordance with the Nurse Practice Act to assigned patients.
• Collaborates with other disciplines of the health care team to ensure the delivery of safe, timely, and appropriate quality patient care.
• Performs physical assessments, nursing interventions, pain management, medication administrations, specimen collection, and meets nutritional needs.
• Addresses teaching and discharge needs of the patients assigned.
• Utilizes the nursing process based on evidence-based best practices to meet patient care needs.
• Evaluates and adjusts the nursing care process as necessary with consultation, and confers with appropriate resource personnel when making ethical/legal/medical dilemma decisions.
• Floats to provide care in both the home health and hospice settings, with assignments throughout Orange County that may change daily.
• For Behavioral Health Nurse (RN): Utilizes medical/psychosocial model to individualize care plans based on physician orders for patients with depression, anxiety, or dementia, aiming to keep patients in their homes and out of the hospital.
• Performs comprehensive behavioral health evaluations of mental status, mood, and behavior.
• Performs other duties as assigned.

Requirements

• Current licensure as a Registered Nurse in the State of California.
• Current Driver’s License and an automobile that is insured in accordance with state/organization requirements and is in good working order.
• Current BLS certification prior to clinical rotation.

Qualifications

• Home Health: One (1) year of home health care experience preferred.
• Hospice: Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred.
