Home Health Nurse (RN)

Responsibilities

• Conducting comprehensive assessments of clients' health status and needs.
• Developing and implementing individualized care plans in collaboration with clients, their families, and other healthcare professionals.
• Administering medications, treatments, and therapies as prescribed.
• Monitoring clients' vital signs and health indicators.
• Performing nursing procedures such as oxygen support, ostomy and catheter care, medication management, and wound care.
• Providing assistance with activities of daily living, such as bathing, dressing, and grooming.
• Educating clients and their families on health management and disease prevention.
• Collaborating with physicians and other healthcare professionals to ensure optimal patient care.
• Maintaining accurate and timely documentation of patient interactions and care provided.
• Supervising Certified Nurse Assistants (CNA); training and delegating specific tasks per agency policy.
• Communicating effectively with clients, families, and interdisciplinary team members.

Requirements

• Current RN license in the state of Colorado.
• Minimum of 3 years of nursing experience, preferably in home care or geriatrics.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal abilities.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Compassionate and patient-centered approach to care.
• Reliable transportation and willingness to travel to clients' homes.
• CPR certification (required).
• Experience with electronic health records (preferred).
