Responsibilities
- Conducting comprehensive assessments of clients' health status and needs.
- Developing and implementing individualized care plans in collaboration with clients, their families, and other healthcare professionals.
- Administering medications, treatments, and therapies as prescribed.
- Monitoring clients' vital signs and health indicators.
- Performing nursing procedures such as oxygen support, ostomy and catheter care, medication management, and wound care.
- Providing assistance with activities of daily living, such as bathing, dressing, and grooming.
- Educating clients and their families on health management and disease prevention.
- Collaborating with physicians and other healthcare professionals to ensure optimal patient care.
- Maintaining accurate and timely documentation of patient interactions and care provided.
- Supervising Certified Nurse Assistants (CNA); training and delegating specific tasks per agency policy.
- Communicating effectively with clients, families, and interdisciplinary team members.
Requirements
- Current RN license in the state of Colorado.
- Minimum of 3 years of nursing experience, preferably in home care or geriatrics.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal abilities.
- Ability to work independently and collaboratively as part of a multidisciplinary team.
- Compassionate and patient-centered approach to care.
- Reliable transportation and willingness to travel to clients' homes.
- CPR certification (required).
- Experience with electronic health records (preferred).
Qualifications
