Home Health Nurse

About the Role

This position involves providing direct nursing care outside hospital settings, primarily in a home healthcare environment for patients across all age groups, including neonates, pediatrics, adolescents, adults, and geriatrics.

Responsibilities

• Assess patient physical, psychological, social, and environmental needs to develop personalized care plans.
• Administer medications, perform treatments, and coordinate ancillary services to meet treatment goals.
• Supervise nursing staff and report patient status changes to the healthcare team.
• Evaluate patient responses, modify care plans accordingly, and ensure safety protocols are followed.
• Establish rapport with patients, families, and healthcare professionals, and utilize community resources.
• Participate in ongoing professional development and uphold patient safety policies.

Requirements

• Graduate of an accredited nursing program with RN licensure.
• Minimum of one year of relevant nursing experience.
• CPR/BLS certification within 30 days of hire.
• Ability to perform sedentary work, involving sitting, walking, and standing as needed.
