Lakeview Village Home Health is searching for caring, dedicated, and trustworthy RN or LPN to join our team! As a Nurse, you will provide comprehensive nursing services to residents and their families in a community setting, with an emphasis on the promotion and maintenance of health, early detection, and restoration measures. This care is provided under the leadership of a RN Case Manager, but as a staff nurse, you will be one of the residents’ closest advocates.
Job Description
- Completes an initial OASIS assessment of the residents and their family to ascertain the level of in-home care needs, including their physical, emotional, and mental needs
- Aids the primary care physician in developing a care plan, maintains appropriate communication with the physician throughout the course of treatment
- Implements a plan of care under the supervision of the Nursing Case Manager, Home Health Administrator, and the Primary Care Physician. Participates in resident care conferences as needed
- Follows universal precautions, infection control procedures, and safety/sanitation standards of practice
- Provides frequent leadership for Home Health Aides by instructing and evaluating care provided
- Provides residents with the appropriate treatment as directed by the primary care physician
- Thoroughly and completely records all treatments in the appropriate charting system to ensure the continuity of care, maintenance of accurate legal documents, and caring, professional services
- Provides residents and their families with education on appropriate medical self-care skills
- Ensures that equipment and other necessary items and/or services are made accessible for residents
- Ensures that residents’ living spaces are maintained in a clean, safe, and orderly manner
- Participate in on-call duties as directed by the Home Health Administrator/Director of Nursing
- Attends and participates in in-service educational classes and on-the-job training programs
- Develops and maintains a good working rapport with inter-department personnel, other departments within the facility, and off-campus contacts to ensure that departmental programs can be properly maintained to meet the needs of the facility and residents
Qualifications
- Must be a graduate of an accredited School of Nursing
- Must possess a current, unrestricted LPN or RN State Nursing license in the State of Kansas
- Must have current CPR certification
- Prior Home Health experience is preferred
- Must have a reliable method of transportation and a valid State driver’s license
- Demonstrates good communications skills, including but not limited to effective reading, writing, speaking, and analytical skills, with a fluent understanding of English
- Must demonstrate both precision and accuracy in the completion of duties each day
- Demonstrates an empathetic approach to the elderly
- Able to work independently, prioritize tasks appropriately, and exercise sound judgement where procedures cannot be standardized
- Able to administer and follow straightforward written or verbal instructions
- Able to work overtime, holidays, and weekends as assigned
