About the role

We are currently seeking Nurses who believe in our mission to help patients live out their best lives at home. The role involves coordinating care and ensuring quality care delivery within the Agency’s scope of services, policies, and procedures, as well as adhering to state and federal laws and regulations.

Responsibilities

• Coordinating care and ensuring quality care delivery
• Providing effective and quality home care services
• Assessing the continuing needs of the patient and performing initial and ongoing patient assessments
• Participating with a team of RNs, LPNs, and Home Health Aides to provide home care services
• Promoting personal safety and maintaining a safe environment for patients and coworkers
• Performing job duties in accordance with Agency policies, procedures, and professional standards

Requirements

• Graduate of an accredited school of professional nursing
• Home Care Experience preferred
• Demonstrates competency with technical nursing skills according to personal and legal scope of practice
• Communicates effectively and in accordance with Agency requirements

$28.00 - $40.00 per hour

Schedule

Part Time, Fridays only: 7:15AM-5:15PM
