Home Health Nurse
About the Role
Coordinates the total Plan of Care for the patient in the Home Care setting, assessing, planning, implementing, and evaluating. Provides leadership for the Health Care team by working cooperatively with other team members in maintaining standards of professional nursing practice in the Home Care setting.
Responsibilities
- Assess, plan, implement, and evaluate patient care plans in a Home Care setting.
- Lead and collaborate with the Health Care team to maintain professional nursing standards.
Requirements
- Associate’s Degree required.
- Current nursing license required.
- 1-5 years of relevant experience required.
Schedule
- Full-time, bi-weekly standard hours: 80.25.
- Work hours: 8:00 AM – 4:30 PM.
- Weekend/holiday requirement: Yes.
- On-call required: Yes, with rotation.
- Work arrangement: Onsite.
Pay
Salary range: $43.49 - $54.26 per hour. Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
