About the Role

As a Home Health RN Case Manager, you will coordinate and deliver individualized care to patients in their homes.
You’ll work independently while collaborating with a supportive interdisciplinary team, helping patients recover, maintain independence, and achieve their health goals.
Responsibilities
- Develop and manage personalized care plans based on each patient’s unique needs
- Perform comprehensive assessments and ongoing evaluations
- Provide skilled nursing care, treatments, and patient education
- Care for patients’ physical, emotional, and social needs
- Coordinate care with physicians, therapists, and other healthcare professionals
- Support patients and families in understanding diagnoses, medications, and care plans
- Maintain accurate documentation and meet all regulatory standards
- Monitor progress and adjust care to ensure the best outcomes
Requirements
- Active RN license in the state of practice
- Previous home health or case management experience preferred
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal abilities
- Ability to work independently while thriving in a team environment
