Home Health Medical Social Worker - PRN

Role Overview: The Medical Social Worker collaborates with an interdisciplinary care team to assess and address patients' emotional, social, and practical needs in a home health setting. They develop personalized care plans, evaluate patient and family responses, and coordinate community resources to facilitate recovery and well-being. Key Responsibilities Assess patients’ social, emotional, and financial situations related to their health conditions.Identify and connect patients with appropriate community resources to support their recovery.Communicate effectively with patients, families, and healthcare team members.Participate in care coordination and provide guidance to other team members.Supervise and evaluate the Social Work Assistant to ensure compliance with standards.Maintain thorough documentation of patient interactions and care activities. Qualifications & Skills Minimum one year of healthcare social work experience, preferably in home health or hospice.Licensure as required by state law.Valid driver’s license, reliable transportation, and proof of CPR certification.Experience with discharge planning and resource referral.Strong organizational, communication, and stress management skills.Fluent in English, with flexibility in work hours and travel.Commitment to ongoing education and licensure maintenance.
