Home Health Medical Social Worker - Per Diem

Role Overview: The Medical Social Worker provides comprehensive social and emotional assessments for home health patients, developing personalized care plans to address their needs. They evaluate patients' relationships, financial resources, and community support systems, coordinating with the interdisciplinary team to ensure optimal care. The role includes supervising the Social Worker Assistant, maintaining documentation, and facilitating access to community resources to promote patient recovery. Responsibilities Assess patients' social and emotional well-being related to their illness or injury.Identify and connect patients with community resources to support recovery.Communicate effectively with patients, families, and healthcare team members.Participate in care coordination and interdisciplinary team activities.Supervise and evaluate the Social Worker Assistant’s performance.Maintain accurate documentation of patient interactions and care plans. Qualifications & Skills Minimum of one year of healthcare social work experience, preferably in home health or hospice.Valid licensure as a social worker in the practice state.Reliable transportation, valid driver’s license, and proof of CPR certification.Knowledge of discharge planning, community resources, and medication administration.Strong organizational skills, ability to prioritize, and work under stress.Fluent in English and flexible to work hours and travel locally.Commitment to ongoing licensure and continuing education. Additional Requirements: This role involves patient-facing duties, TB screening, and adherence to driver safety policies. Compensation ranges from $65,000 to $88,600 annually, with benefits supporting overall well-being.
