Role Overview: The Medical Social Worker collaborates with the interdisciplinary care team to evaluate and address the emotional, social, and care needs of home health patients. They develop personalized care plans, assess patient and family dynamics, and facilitate access to community resources to support recovery and well-being. Key Responsibilities Assess patients' social and emotional states related to their health conditions.Identify and coordinate community resources such as housing, legal aid, and financial assistance.Communicate effectively with patients, families, and healthcare team members.Monitor ongoing patient and family needs, adapting care plans accordingly.Participate in care coordination and act as a resource within the team.Supervise and evaluate the Social Work Assistant to ensure compliance with standards.Maintain accurate documentation of patient interactions, care plans, and progress notes. Qualifications & Skills Minimum of one year of healthcare social work experience, preferably in home health or hospice.State licensure as required; valid driver’s license, auto insurance, and reliable transportation.Current CPR certification.Experience with discharge planning and accessing community resources.Knowledge of medication administration and healthcare regulations.Strong organizational, communication, and stress-management skills.Fluent in English, with flexibility for local travel.Commitment to ongoing education and maintaining licensure. Additional Details: This patient-facing role involves TB screening and driver safety requirements.
