Full-time, weekday position serving the New Kent, Charles City, Lanexa, and Providence Forge territory.

Pay

$30.00/hr to $35.00/hr, with a $5,000 sign-on bonus.

Responsibilities

• Verify plan of care with the RN prior to each visit
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them as soon as possible when patient condition changes
• Teach patients and caregivers self-care techniques
• Check vital signs
• Review all medications and perform medication reconciliation at each visit
• Perform specialized procedures ordered by physician (e.g., foley catheter change)
• Prepare equipment and materials for treatments using aseptic technique
• Complete documentation timely and accurately

Requirements

• One year of clinical LPN experience
• Current LPN license, specific to the state(s) assigned to work
• Current CPR certification
• Valid driver’s license, reliable transportation, and liability insurance
