Responsibilities

• Provide home-based care under RN supervision to patients with illnesses, injuries, and chronic diseases, often following hospital, rehab, or nursing facility discharge
• Work with physicians, RNs, and CNAs to deliver quality patient care
• Examine patients, complete assessments, document progress, and report changes to physicians
• Educate patients on care plans, medications, therapy, diet, and exercise
• Coach family members on care plans, medication, and home safety
• Collaborate with interdisciplinary teams to coordinate care and ensure goal attainment and discharge planning

Requirements

• Graduate of a practical (vocational) nursing program for LPN with a current GA LPN license
• Minimum of 1 year LPN experience in home healthcare
• HCHB charting experience highly preferred
• CPR certification and negative PPD screening or chest X-ray
• Driver’s license, reliable transportation, and auto insurance
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills, and compassion
• Ability to pass federal and state required criminal/abuse background checks and drug screen
