Home Health Licensed Practical Nurse (LPN)

Role: Hospice LPN Float providing practical nursing care in patients' homes within the Mt. Vernon area. Responsibilities: Monitor patient conditions and document care accurately.Administer medications and treatments as prescribed.Assist with symptom and pain management.Coordinate with hospice nurses and interdisciplinary team members.Communicate updates to patients, families, and team members.Support patients and caregivers in understanding care plans. Qualifications and Skills: Compassionate and patient-focused approach.Strong communication and teamwork skills.Ability to assess patient needs and adapt care accordingly.Knowledge of hospice care practices and procedures.
