What You'll Do

• Delivering care in accordance with physician orders, delegated nursing responsibilities, and the established Plan of Care
• Making sure to monitor patient conditions and report significant changes to the RN Case Manager and healthcare team
• Aiding patients and caregivers with education and self-care techniques
• Implementing patient safety plans and utilizing available community resources
• Collaborating with the interdisciplinary care team to ensure exceptional patient outcomes
• Accurately and timely documenting clinical findings, nursing interventions, and medication updates
• Identifying opportunities to improve patient outcomes and reduce avoidable hospitalizations and emergency room visits
• Providing compassionate, professional support to patients, families, physicians, and colleagues

Qualifications

• Graduate of an accredited Certificate, Diploma, or Associate Degree Nursing program
• Current Massachusetts Licensed Practical Nurse (LPN) license
• Minimum of one (1) year of clinical nursing experience
• Valid driver's license, reliable transportation, and current auto insurance
• Ability and willingness to travel throughout the assigned territory
• Prior home health or hospice experience preferred

Physical Requirements

• Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
