Home Health Field Registered Nurse (RN)

About the role

Provide direct patient care in a home health setting. Perform assessments, develop care plans, administer medications, educate patients and families, and coordinate with other healthcare providers.

Responsibilities

• Assess patient conditions and needs
• Develop and implement care plans
• Administer medications as prescribed
• Educate patients and families on self-care and treatment
• Coordinate with physicians and other healthcare professionals
• Maintain accurate medical records

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• At least 1 year of relevant clinical experience
• Passing scores on the NCLEX-RN exam

Qualifications

• Strong communication skills
• Ability to work independently and as part of a team
• Flexibility to travel within the designated service area

Skills

Pay
