Home Health Field Registered Nurse (RN)

About the role

Provide direct patient care in a home health setting. Perform assessments, develop care plans, administer medications, and educate patients and their families.

Responsibilities

• Assess patient needs and develop individualized care plans
• Administer prescribed medications and treatments
• Monitor patient progress and report changes to healthcare providers
• Provide education and support to patients and families
• Ensure compliance with regulatory standards and policies

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• At least 1 year of acute care nursing experience
• Basic Life Support (BLS) certification

Qualifications

• Strong clinical skills and knowledge of home health care practices
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively
• Flexibility to travel within the assigned service area
