Home Health Coordinator – Registered Nurse
About the role
Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator‑RN. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards, interacts with other facility staff, actively participates in interdisciplinary team meetings, conducts initial and periodic assessments, administers nursing care to ill, injured, convalescent or disabled participants, advises participants on health maintenance and disease prevention, and administers medication when needed.
Responsibilities
- Coordinate and supervise all nursing care for participants per program standards
- Interact with other facility staff
- Participate in interdisciplinary team meetings
- Conduct initial and periodic assessments
- Administer nursing care to ill, injured, convalescent or disabled participants
- Advise participants on health maintenance and disease prevention
- Administer medication as needed
Requirements
- Associate’s Degree in Nursing (required)
- Active RN license (required; must be active in VABLS within 90 days)
- Minimum 1 year of nursing experience (required); home health experience highly preferred

Pay
Base pay range for full‑time employment: $87,214.40 – $126,900.80 annually. Additional compensation may be available, such as shift differentials, on‑call pay, shift premiums, extra‑shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
Schedule
Full‑time day shift (First shift). Mileage reimbursement is provided for all miles driven between patient visits.
