Home Health Coordinator - Registered Nurse

About the role

Sentara Home Care is seeking to hire a qualified individual to join our team as a Home Health Coordinator‑RN. The RN Home Health Coordinator coordinates and supervises all nursing care for participants according to program standards, interacts with other facility staff, actively participates in interdisciplinary team meetings, conducts initial and periodic assessments, administers nursing care to ill, injured, convalescent or disabled participants, advises participants on health maintenance and disease prevention, and administers medication if needed.

Responsibilities

• Coordinates and supervises all nursing care for participants according to program standards.
• Interacts with other facility staff.
• Actively participates in interdisciplinary team meetings.
• Conducts initial and periodic assessments.
• Administers nursing care to ill, injured, convalescent or disabled participants.
• Advises participants on health maintenance and disease prevention.
• Administers medication if needed.

Requirements

• Associate's Degree in Nursing (required).
• Active RN licensure (required); must be active in VABLS within 90 days of hire.
• Minimum 1 year of nursing experience (required).
• Home health experience (highly preferred).

Pay

The base pay range for full‑time employment is $87,214.40 – $126,900.80 annually. Additional compensation may be available for this role, such as shift differentials, on‑call pay, shift premiums, extra‑shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs. (Note: If an annual salary is posted, it is based on a full‑time colleague working 2,080 hours annually.)
