About the role
As a Home Health RN, you will:
- Provide skilled, compassionate nursing care in accordance with the nursing process, physician orders, and the patient’s individualized Plan of Care (POC).
- Assess and monitor patient condition, regularly re-evaluating needs.
- Administer medications, treatments, and specialized procedures within scope of practice, training, and demonstrated competency.
- Communicate changes in patient condition and care needs with physicians, supervisors, patients, and families/caregivers to promote continuity of care.
- Educate patients and families/caregivers on medications, treatment plans, disease management, safety, and self-care to support independence.
- Complete accurate, timely, and thorough clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
- Promote a safe, efficient care environment while supporting activities of daily living, appropriately delegating/supervising staff, and maintaining professional knowledge through ongoing education.
Requirements
- Graduate of an accredited school of professional nursing (BSN preferred).
- Currently licensed to practice as an RN in the state of practice.
- If IV or ET certified, currently certified and recognized by the state(s) where practicing as an RN.
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
- Possess and maintain a valid driver’s license.

Pay
This position is based on a pay-per-visit compensation model. Pay per visit ranges from $110–$180, commensurate with experience and depending on the type of visit.
