Registered Nurse - Case Manager (Home Health)

About the role

We are seeking a skilled Home Care Registered Nurse (RN)/Field Case Manager to provide independent and team-based home care nursing across multiple counties.

Responsibilities

• Assessing and coordinating patient care plans
• Performing skilled nursing assessments and complex wound care
• Managing PICC lines, ports, blood draws, TPN, antibiotics, catheters, drains, and tracheostomy care
• Monitoring health conditions, educating patients and families to promote wellness
• Acting as a team leader, supervising Home Health Aides, and ensuring regulatory compliance
• Completing documentation, OASIS assessments, and communicating with healthcare providers

Qualifications

• Current IL RN license, BLS certification, valid driver's license, and proof of auto insurance
• At least 1 year of nursing experience
• Knowledge of Medicare benefits, documentation standards, and regulatory guidelines
• Strong case management, communication, and supervisory skills
