About the role

Responsible for overseeing and coordinating home health clinical operations and patient care processes. Works under the direction of leadership to implement operational plans, ensure patient-centered care delivery, and support quality outcomes.

Responsibilities

  • Ensure high-quality customer service and patient-centered care delivery across home health services
  • Oversee daily clinical operations, ensuring adherence to agency policies and care standards
  • Assign patients to clinical staff and manage schedules to ensure appropriate care coverage
  • Monitor and support care plan development, implementation, and ongoing evaluation
  • Promote culturally competent, respectful, and individualized patient care
  • Accurately and timely document clinical work aligned with regulatory requirements
  • Facilitate interdisciplinary team meetings to support care coordination and communication
  • Conduct supervisory field visits and evaluate clinical staff performance
  • Follow up with patients and healthcare partners for satisfaction
  • Maintain clinical records, reports, and quality metrics for performance evaluation
  • Ensure compliance with all federal, state, and local home health regulations
  • Act as liaison between physicians, patients, families, and healthcare partners
  • Support onboarding, orientation, and competency validation of new clinical staff
  • Lead staff development initiatives and promote ongoing clinical education
  • Support quality improvement and performance evaluation initiatives
  • Act on behalf of clinical leadership in their absence when required

Requirements

  • Registered Nurse (RN) license in Pennsylvania
  • Minimum 2 years of home health field experience
  • Bachelor's degree in Nursing preferred
  • Prior supervisory or management experience in home health preferred
  • Valid driver's license and reliable transportation
