About the role
We are seeking a dedicated RN Case Manager who genuinely enjoys engaging with seniors and delivering top-tier care in a home setting. In this role, you will partner with a multidisciplinary team to design and carry out customized care plans while maintaining clear, ongoing communication with clients, families, caregivers, and nursing staff. Strong problem-solving abilities and seamless collaboration with our administrative team are essential. As the care manager, you will oversee and streamline service delivery across a network of caregivers, nurses, therapists, and external partners. This position calls for strong clinical judgment and proven experience navigating private-pay client dynamics and family relationships.
Option to work one weekend day and have one weekday off.
Responsibilities
- Design and carry out customized care plans in collaboration with a multidisciplinary team
- Maintain clear, ongoing communication with clients, families, caregivers, and nursing staff
- Oversee and streamline service delivery across caregivers, nurses, therapists, and external partners
- Apply strong clinical judgment in managing private-pay client dynamics and family relationships
Requirements
- Active California RN License
- Minimum 2-3 years of home health nursing experience
- Experience managing private pay clients
- Master in Nursing (preferred)
- Strong computer skills
- Ability to multitask, drive, and take action
- Strong organizational and communication skills
- Pass Live Scan Fingerprint Clearance
- Valid driver’s license and proof of insurance
- Cell phone with internet access
- Proof of negative TB results
- Willing to work one weekend a month as back-up on call

Pay
Salary $120,000+ based on experience. Potential for team bonus.
