About the Role
Acclaim Rehab and Nursing Center is seeking a dependable and compassionate Home Health Aide (HHA) to assist residents during clinic and dialysis appointments while ensuring their safety, comfort, and well-being.
Key Responsibilities
- Escort residents to and from clinic and dialysis appointments
- Arrive one hour prior to scheduled appointments
- Assist residents in preparing for appointments
- Ensure residents receive snacks and required documentation
- Monitor residents to prevent falls or incidents
- Report all incidents immediately to nursing leadership
- Communicate with clinic staff and facility regarding resident status
- Maintain professionalism and follow all facility policies
Requirements
- Current New York State HHA certification (required)
- High school diploma or equivalent (preferred)
- Strong communication skills and reliability
- Nursing home experience (preferred)
Schedule & Pay
- Schedule: Part-Time | 7:00 AM – 3:00 PM
- Pay Rate: $18–$20/hour
