Responsibilities
- Assists with rising patient care needs
- Provides excellent personal care to patients to maximize satisfaction according to Aide Plan of Care
- Maintains safe environment in patient’s home
- Manages visiting caseload productivity within the appropriate expected time frame
- Documents care provided to meet regulatory requirements in Electronic Medical Record (EMR)
- Consults with professional staff on patient problems and is involved with long and short-term goals that are consistent with organization’s vision, mission, values and goals
- Completes continuing education and competency requirements within designated time frames as specified by Medicare conditions of participation
- Provides and/or assists with personal hygiene activities
- Utilizes preventive skin care measures
- Performs special procedures/treatments
Qualifications
- A High School Diploma or GED is required
- One year of HHA/CNA experience is preferred
- A valid Maryland Board of Nursing Certified Nursing Assistant (CNA) certification is required
- A valid driver’s license with agency insurance requirements is required
