Suburban Home Health is a jointly owned venture between several Suburban Health Organization hospital systems in central Indiana. We work as an extension of the trusted hospital consortiums in your community, with the goal of improving care across the continuum for our homebound patients. Responsibilities

• Provides personal care activities of daily living such as bathing, shampooing, oral hygiene, skin care/nail care, shaving, and dressing.
• Communicates changes in patient status to the supervising Registered Nurse (RN) in a timely manner.
• Reports patient complaints to the RN.
• Creates and submits legible, accurate, and timely documentation.
• Performs duties in compliance with agency policies and procedures and local, state, and federal regulations.
• Demonstrates attention to details for patient's safety and safety of their home environment.
• Reports unsafe conditions in the home.
• Practices effective infection control and adheres to infection control policies and procedures.
• Practices safe lifting and transferring of patients.
• Communicates clearly and positively with patients, families, team members, and others.
• Displays a caring, patient-first behavior.
• Keeps patients and/or family informed regarding visit time and schedule changes.
• Attends in-service programs to meet compliance requirements.
• Maintains patient confidentiality in compliance with HIPAA.
