About The Job
MedStar Health is seeking a PRN - As Needed Day Shift Home Health Aide Per Visit to join their team. This role requires a Virginia CNA license, an active driver's license, proof of car insurance, and reliable transportation. Ideal candidates should have experience as a Certified Home Health Aide and possess excellent communication skills.
Primary Duties
- Attends Agency regional office and team meetings as required.
- Attends in-service educational programs.
- Demonstrates flexibility in adjusting to changes in procedures and daily schedule.
- Cleans direct patient care area, as necessary.
- Identifies safety hazards in the home and reports them to the team's Operations Director or primary case-manager.
- Reinforces with patient/family ways to remove identified safety hazards in the home.
- Documents services in accordance with Agency medical records and timekeeping policies.
- Documents patient progress reports clearly and concisely for multidisciplinary case conference.
- Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams.
- Participates in meetings and on committees and represents the department and agency in community outreach efforts.
- Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.
- Performs personal care activities (bathing, dressing, oral hygiene etc.)
- Performs other related activities (light meal preparation, personal laundry etc.)
- Affords assistance with transfers and range of motion exercise programs as instructed by professional staff.
- Performs procedures as delineated on HHA (Home Health Agency) Skills Competency Checklist as instructed by professional staff.
- Recognizes signs of emotional distress (such as anger and depression).
- Encourages patient mobility and socialization during visits.
- Recognizes and responds to special needs of patients/families experiencing terminal illness.
- Reports identified alterations in patient behavior or mental status to primary case manager or the team's Operations Director.
- Reports identified patient care or family health or social problems to the team's Operations Director or primary case manager.
- Reports all problems in carrying out assigned activities to primary case manager or the team's Operations Director.
- Collaborates with case manager or professional staff in the revision and implementation of the Home Health Aide plan of care.
- Informs the team's Operations Director of the need to change the patient's visit schedule.
Qualifications
- Ability to read, write, and perform arithmetic calculations.
- High School Diploma or GED preferred.
- Home care experience preferred.
- CNA (Certified Nursing Assistant) in Virginia.
- District of Columbia, Maryland, Virginia, or any combination as required based on work location(s).
- CPR (Cardiac Pulmonary Resuscitation).
- Completion of MedStar Health's home health aide wound diabetes and CHF competencies after hire.
- Valid State Driver’s License in good standing with a dependable vehicle for transportation.
Pay
This position has a hiring range of USD $19.55 - USD $34.25 /Hr.
