About the Position
As a Home Health Aide (C.N.A.) with The Hub on Smith, you'll play a key role in providing compassionate, personalized care to individuals in their homes. Your responsibilities will include assisting with daily living activities, such as bathing, dressing, and meal preparation, as well as offering companionship and support. You'll help create a safe and comfortable environment that promotes independence and well-being. Prior experience in Home Health is beneficial but not required— we value your dedication to making a positive impact.
ESSENTIAL JOB FUNCTIONS
- Direct Care Responsibilities
- Regularly provides personalized care, homemaking, and respite services to dependent adults within their homes.
- Affords assessments of individual needs, preferences, and adapts care accordingly.
- Assists individuals with transfers and positioning, exerting up to 50 pounds force unaided.
- May assist individuals with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).
- Communication and Reporting
- Interacts with individuals respectfully and effectively communicates with them.
- Frequently engages in verbal communication and exchanges of information with others.
- Promptly reports any health or safety concerns related to individuals or the program to the supervisor.
- Training and Compliance
- Completes comprehensive orientation and ongoing training as directed by the organization.
- Proficient and willing to perform CPR and Basic First Aid.
- Annual Tuberculosis (TB) assessment is mandatory.
- Maintains strict confidentiality regarding sensitive information.
- Administrative and Other Responsibilities
- Undertakes additional responsibilities as delegated.
- Drives to home settings using personal transportation, holding a valid driver's license and insurance.
- Performs additional tasks and responsibilities as needed to support the organization's goals and operations.
- Physical Requirements
- Demonstrates a full range of physical motion as required by the tasks.
- Regularly lifts, transfers, and positions objects weighing up to 50 pounds without assistance.
- Utilizes mechanical lifts to lift objects weighing more than 100 pounds.
