Key Responsibilities
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and eating.
- Help with mobility, including transferring clients from bed to wheelchair or assisting with walking.
- Provide companionship and engage in activities to support mental and emotional well-being.
- Aid with light housekeeping, meal preparation, and laundry as needed.
- Monitor and report any changes in the client’s physical or emotional condition to the appropriate supervisor or healthcare team.
- Help with medication reminders, ensuring medications are taken as prescribed.
- Follow care plans developed by healthcare professionals to ensure clients' needs are met.
- Maintain accurate documentation of services provided and any changes in client conditions.
- Monitor vital signs and appropriately report signs/symptoms of concern or illness to Care Manager or Executive Director of Clinical Services.
- If patient has gone to hospital or doctor for care/concerns, report changes to medication
Qualifications
- High school diploma or equivalent.
- Certification as a Home Health Aide.
- Caregiver must be compassionate, patient, and able to build trusting relationships.
- Must have the ability to perform light housekeeping and other daily living tasks.
- Must maintain confidentiality and adhere to privacy standards (HIPAA).
- Must have reliable transportation to travel to clients’ homes.
Physical Requirements
- Ability to lift up to 25 pounds.
- Ability to stand, sit, bend, and kneel for extended periods.
- Ability to assist clients with mobility and transfers.
While performing the duties of this job, the employee is regularly exposed to an office environment in which the noise level is usually low to moderate. The employee is occasionally exposed to outside weather conditions as well as individual residences in which the noise levels are usually moderate.
