Home Health Aide

About the role

The Home Health Aide provides home health patient/resident care, administers medication, performs treatments, and supports the care team under licensed nurse supervision. This position covers the western region (LaMoure, Ellendale area).

Responsibilities

• Provide home health patient/resident care and perform medication administration and various treatments under the direction of the licensed nurse as required.
• Complete assigned tasks to ensure timely, efficient and safe patient/resident care.
• Communicate and interact with patients, residents, families and other health team members.
• Perform initial and ongoing data collection on home health patients/residents under the direction of the Registered Nurse/Licensed Practical Nurse.
• Implement plan of care developed by the Registered Nurse based on patient/resident physical, psychosocial, cultural, ethnic and religious/spiritual needs.
• Provide direct patient/resident care and report condition or status changes accurately and timely to the RN or LPN.
• Demonstrate time management and prioritization of tasks.
• Document results of patient/resident care within established policies, procedures and scope of practice.
• Follow facility policies on medication administration and treatments under the direction of a licensed nurse.
• Promote the Benedictine Mission and Core Values of Hospitality, Stewardship, Respect and Justice by bringing the Mission and Core Values into day‑to‑day activities.
• Perform other duties, tasks and/or projects as assigned.

Qualifications

Required:

• Be in good standing on the State Nursing Assistant registry or appropriate entity.
• Complete required training for medication administration and various treatments.
• Hold a valid Driver’s License.
• Maintain an acceptable driving record that meets Benedictine requirements.

Preferred:

• Previous home health, hospital, or nursing home experience.
• BLS Certification.
