Requirements

• Minimum 1 year experience as an HHA
• Valid NYS HHA Certificate
• Proof of eligibility to work in the U.S.
• Valid driver’s license (REQUIRED)
• Physical exam (within 1 year)
• PPD or QuantiFERON (within 1 year) or chest X-ray (within 5 years)
• MMR immunizations (within 10 years)

I also certify that to the best of my knowledge, I do not pose any risk to myself or others and that all information that I have submitted or stated is accurate.
