Home Caregiver (HCA/CNA) – Seattle Area

About the role

As a Caregiver, you will follow the client’s individualized care plan, assisting clients with daily living activities at their home or care facility. Your commitment will ensure clients are satisfied with the service and quality of care they receive.

Responsibilities

  • Perform all duties per the client’s individualized care plan as directed by the In-Home Care Supervisor or Manager.
  • Assist with activities of daily living (ADL) as directed by the care plan, including but not limited to:
    • Chore Services: housekeeping
    • Medication Assistance (as delegated)
    • Personal Care: bathing, showering, toileting, grooming, dressing
    • Meal Preparation
    • Shopping and Transportation
    • Repositioning/Transfers/Lifting
    • Standby Assistance
    • Companionship
  • Communicate with the care management team about any changes in the client’s condition, requests for scope of care outside the plan, or client requests outside the scope of your license or credentials.
  • Provide emotional and social support as required by client preference or the care plan.
  • Actively engage clients in a manner that promotes their independence and maintains their dignity.
  • Accurately maintain client records in a timely manner, including documentation of medication distribution, leisure activities, reportable incidents, client observations, and changes to health or emotional condition.
  • Immediately report all changes in condition to the Client Care Manager and care management team.
  • Ensure the client’s environment remains safe, clean, and orderly.
  • Perform general housekeeping duties and maintain cleaning schedules for client’s laundry, bedrooms, dining area, living space, bathrooms, kitchen, and other areas as directed.
  • Safely transport clients to and from appointments, errands, and activities as directed by the plan of care.
  • Assist clients with ambulation and mobility, including transfers in and out of bed, chairs, wheelchairs, and standby assistance.
  • Adhere to proper safety protocols and company procedures in emergency situations.
  • Assist client with physical therapy activities as directed by the plan of care.
  • Respond promptly and positively to client requests for assistance.
  • Provide excellent hands-on care and bedside manner.
  • Regularly report to all scheduled shifts and accurately record all time worked via Telephony or Clear Care phone applications.
  • Maintain professionalism and confidentiality while representing Fedelta Home Care.
  • Adhere to HIPAA privacy practices and maintain strict confidentiality of sensitive information.
  • Other duties, responsibilities, and activities may change or be assigned at any time with or without notice.

Requirements

  • Strong time management, interpersonal skills, patience, listening skills, and knowledge of generally accepted care standards.
  • Ability to communicate effectively in English, both verbally and in writing, through various channels including in-person, phone, email, and text messages.
  • Demonstrated experience and tact in communicating with diverse populations, including the ability to recognize and understand differences in needs, experiences, values, lifestyles, and ages.
  • Ability to maintain professionalism and customer service standards in stressful situations.
  • Regular local travel and reliable transportation to assigned client locations.

Qualifications

  • Certified Nursing Assistant (C-NA), Home Care Aide (HCA), Home Health Aide (HHA), or Nursing Assistant Registered (NAR).
  • Minimum of six months of industry-related experience.
  • Current Washington State CNA, HCA, HHA, or NAR credential in good standing, or the ability to obtain required certification within timelines established by Washington State regulations (Chapter 246-980 WAC).
  • RN Delegation certification is strongly preferred but not required.
  • Valid Washington State driver’s license, acceptable driving record, and proof of current auto insurance for employees who operate a vehicle.
  • Ability to successfully complete all pre-employment and ongoing background check requirements.

Skills

  • Demonstrated experience in Microsoft Office Suite (Word, Excel, Outlook).
  • Experience with or technical aptitude to proficiently use healthcare programs such as WellSky Personal Care.

Physical Demands

  • Frequently required to lift and/or move up to 20 pounds.
  • Occasionally required to lift and/or move up to 50 pounds when assisting with direct patient care.
  • May seldom be required to lift and/or move more than 100 pounds assisted when providing direct patient care.
  • Regularly required to sit, stand, walk, lift, balance, use hands to finger, handle, grasp, or feel; talk and hear; bend and reach with hands and arms; and use repetitive motion of wrists, hands, and/or fingers.
  • Occasionally required to climb ladders or steps, carry, push, pull, or otherwise move objects, including the human body.
  • Specific vision abilities required include close vision, peripheral vision, depth perception, and the ability to adjust focus.
  • Visual acuity necessary for preparing and analyzing data, viewing computer terminals, reading, operating office equipment, operating a motor vehicle, and determining accuracy, neatness, and thoroughness of assigned work.

Work Environment

  • Regularly exposed to both inside and outside environmental conditions while conducting home visits and travel between locations.
  • May occasionally be exposed to atmospheric conditions such as fumes, odors, dusts, mists, or poor ventilation during in-home visits.
  • May be exposed to infectious diseases.
  • The noise level in the work environment is typically low, similar to a traditional home or office setting.
