Apex Rehabilitation is seeking compassionate Home Care Rehabilitation Therapists (Occupational Therapist, Physical Therapist, and Speech Therapist) to join our team in the 10928 area and surrounding communities. This role offers flexible part-time or full-time hours, allowing you to create your own schedule.

About the Role

As a Home Care Therapist, you will enhance clients' independence and quality of life through personalized therapy plans delivered in the comfort of their homes.

Responsibilities

• Conduct in-home assessments to evaluate clients' daily living needs and challenges.
• Develop and implement customized rehabilitation therapy plans to address clients' specific goals.
• Provide therapeutic interventions to improve clients' ability to perform daily activities.
• Collaborate with clients, families, and healthcare professionals to ensure a holistic approach to care.
• Educate clients and their families on adaptive techniques and equipment for increased independence.
• Maintain accurate and detailed documentation of assessments, interventions, and progress.

Requirements

• Valid/active degree from an accredited Occupational Therapy, Physical Therapy, or Speech-Language Pathology program.
• New York State licensure as an OTR, PT, or SLP.
• Strong clinical and communication skills.
• Ability to adapt therapy plans to diverse client needs.
• Compassionate and patient-centered approach to care.
• Valid Driver’s License and/or reliable transportation.
• Valid liability insurance.

Qualifications

• Previous experience in home care or a related field is preferred.
• New graduates with a passion for home care are welcome to apply.
