Home Care Supervisor/Physical Therapist

About the role

Deliver personalized physical therapy services to home-based clients. Conduct initial and ongoing assessments to track progress and adjust treatment plans. Perform fall risk assessments at Assisted Living Facilities (ALFs) and community expos. Supervise client care to ensure best practices and quality outcomes. Represent our organization at ALF events and expos, providing educational presentations. Travel to client locations and events as needed. Supervise Home Care clients and caregivers. Create and update care plans for non-medical clients. Administrative duties including calling PCP offices for orders.

Responsibilities

Requirements

• Licensed Physical Therapist (PT) in the state of Hawaii.
• 1+ years of experience in a clinical or home-based care setting.
• Experience with fall risk assessments (preferred).
• Excellent communication and interpersonal skills to engage clients and the community.
• Valid driver’s license & reliable transportation to travel as needed.

$85,000 - $110,000 per year
