About the role

The Clinical Home Care Nursing Supervisor (Supervisor) is a pivotal leader responsible for the direct supervision of nurses and other vital members of the home health and home-based palliative care team, serving both adult and pediatric populations. This role demands exceptional clinical expertise, visionary leadership, and strategic oversight to ensure the delivery of outstanding patient care.

Responsibilities

  • Strategic Planning: Orchestrates staff scheduling, upholds productivity standards, and approves essential overtime.

  • Nursing Process and Practice: Leads hiring, orientation, and comprehensive training of all team members, fosters a culture of excellence and continuous learning.

  • Quality: Engages in high-level interprofessional problem-solving, spearheads performance improvement initiatives, ensures compliance with regulatory standards, conducts staff competency assessments, and delivers annual performance evaluations.

  • Leadership and Management: Oversees total, comprehensive, continuous, and patient-centered care, delivering direct patient care, ensuring patient satisfaction, and adhering to organizational policies and procedures.

  • Nursing Research: Identifies and implements evidence-based nursing interventions, evaluates their impact on diverse patient populations, and acts as a resource for the home health and palliative care teams.

  • Professional Development: Promotes adherence to organizational policies, champions best practices, and ensures the highest standards of care and professionalism are consistently maintained.

Requirements

  • Registered Nurse with a robust foundation of clinical knowledge, particularly in home-based health services including acute and chronic pediatric care, palliative management of chronic conditions, and compassionate end-of-life care.

  • Exceptional clinical expertise, visionary leadership, and strategic oversight.

  • Exceptional judgment, decisive decision-making abilities, and advanced organizational skills.

Qualifications

  • Graduation from an accredited school of nursing with a Bachelor of Science in Nursing (BSN).

  • At least three (3) years of recent Home Health RN experience.

  • At least one (1) year of recent Home Health and Home-Based Palliative Care RN experience in a leadership role.

  • Valid California driver’s license, with a driving record that meets local policies/procedures, and enrollment in the California Employer Pull Notice Program.

  • Knowledge of current trends in healthcare and interdisciplinary clinical practice.

  • Ability to remain calm under pressure and effective interpersonal skills.

  • Demonstrated accountability and professionalism.

  • Adaptability to change in a positive, professional, and productive manner.

  • Knowledge of performance improvement and evidence-based practice.

  • Basic computer skills.

  • Commitment to excellence in patient-centered care.

  • Knowledge of legal and ethical considerations in healthcare and clinical practice, including confidentiality, patient rights, and documentation standards relevant to position.

  • Ability to assess, plan, implement, and evaluate patient care, taking into consideration protective interventions, development (behavioral and physical), and physiological characteristics specific to the patients.
